How to Get Started: Group Insurance
If you are a business owner looking to obtain a quote for your employees insurance needs we will require the following information:
What are some of the simple rules that employers need to understand?
- To start a group benefits program a minimum of 3 employees are required to participate.
- Employers must offer benefits to all employees in the same class of worker. ie. Clerical, management, factory worker
- Programs maybe co-funded by employers and employees, a minimum of 50/50 sharing is required.
1. Fill out the Request to Quote form
2. Employee Data sheet:
You will need to collect the following information to complete the Employee Data Form:
o Employee Name
o Gender
o Date of Birth
o Marital Status (single or family)
o Smoker or Non
o Occupation
o Earnings
3. 2 years of claims history with your current insurance carrier. Information may be found on line.
4. A current billing statement showing renewed rates and a copy of your billing statement showing this past year’s rates. Information may be found on line.
5. A copy of your current insurance booklet.
